Social prescribing is a free service which aims to support people to take better control over their health and wellbeing.
How social prescribing works
Social prescribing is a free service for Surrey Heath residents that puts you in touch with a wide range of local services and activities in the community to help improve your overall wellbeing.
A member of our social prescribing team will take time to listen to what matters to you and will expertly guide you to access the support and services available.
What we can help with
The team can support you to access the following:
- support with living independently at home
- transport to help with getting out and about
- groups and activities to support with physical and mental health
- leisure and social activities
- advice services including benefits, finance and housing
- support around a disability
- family and carer support services
Who can use this service
This service is available to adults who live in Surrey Heath and/or are registered with a Surrey Heath GP surgery.
